Printed space for information: "No., name, order, remarks, retouched, order finished, reorder". Handwritten in ink, "W. W. Fitzgerald Baby 1712 N Adams". Handwritten in pencil, "in Panel 1.10 [?] 8/2 [?] [/] 1.00 Phone 2596" and a math equation adding 1.25, .50, .35 to equal 2.10.10.
